A Comparative Study of the Clinical Features of Surgical Biliary System Diseases between the Young Age and Old Age Groups


Abstract
This study attempts to compare the clinical features between the young age group(under the 30 years old) and old age group(above the 70 years old) of biliary system diseases which were surgically treated at the department of general surgery,
Chung-Ang
university Young-San hospital. Each group was composed of 100 patients which were chosen randomly from the 1597 consecutive hospitalized cases in this hospital from July 1985 to June 1991.
@ES The results are as rollows:
@EN 1) The young age group was composed of 103 cases(6.4%) and the old age group was composed of 132 cases(8.2%) from the total 1597 cases of surgical biliary system diseased patients.
2) Male to female sex ratio was 1 : 1.8 in the young age group and 1 : 1.9 in the old age group.
3) Most frequent symptoms and signs were RUQ pain(82%), RUQ tenderness(59%) and indigestion(25%) in young age group, RUQ pain (93%), RUQ tenderness(71%) and jaundice(33%) in the old age group.
4) The laboratory data showed, leukocytosis(27%), elevated serum GPT level(21%) and jaundice(18%) in the young age group, elevated serum GPT(44%), leukocytosis(42%) and elevated serum alkaline phosphatase level(38%) I the old age group.
5) The mean period from the onset of symptom development to admission was 20 months among the young age group and 43 months among the old age group.
6) The mean dates from admission to operation were 3.46 days in the young age group and 7 days in the old age group.
7) Mean period of hospitalization was 18 days for the young age group and 26 days for the old age group.
8) The most commonly performed operative procedure was cholecystectomy alone which was perfomed 80% in the young age group and 60% in the old age group.
9) In point of location of stones, gallbladder(71%), CBD(6%) and CBD with IHD(5%) in the young age group, gallbladder(56%), GB with CBD(18%) and CBD(9%) in the old age group.
10) The most common post operative diagnosis was chronic cholecystitis due to stones which was 52% in the young age group and 34% in the old age group.
11) The most common post-operative complication was pleural effusion and atelectasis occurring 3 times in the young age group and 11 times in the old age group.
